In Memory of Phyllis Wolk

It is with great sadness that I announce the passing of Phyllis Grossberg Wolk, founder and former owner of Caring Connections Geriatric Care Management. Phyllis passed away on July 29, 2026,
at Arden Courts, where she had relocated in May of this year.

A highly skilled social worker, Phyllis was a pioneer in the field of Geriatric Care Management. Through her vision, expertise, and unwavering dedication to older adults and their families, she helped shape and advance a profession devoted to enhancing the quality of life of aging individuals. As the founder of Caring Connections, she

established a legacy of compassionate care, advocacy, and support that has touched countless lives.

Phyllis was respected by colleagues, cherished by clients and their families, and admired for her commitment to helping others navigate the challenges of aging with dignity and confidence. Her leadership, wisdom, and kindness left an indelible mark on the profession and on all who had the privilege of knowing her.

Over the years, Phyllis served as a valued mentor to me and to many others at Caring Connections. Her leadership, and unwavering commitment to the field of geriatric care management profoundly influenced both our organization and the many professionals she guided. I remain dedicated to honoring her legacy by continuing the mission she so thoughtfully established and steadfastly advanced.

A Memorial Service and Celebration of Life will be held at Temple Emanu El in Solon on Sunday, September 6, at 12:00 noon. The service will also be livestreamed at https://www.teecleve.org/ for those unable to attend in person.

In lieu of flowers, contributions in Phyllis's memory may be made to the National Council of Jewish Women.
